What We’re Looking For:

• Licensed Physical Therapy Assistant in New Jersey.
• Degree from an accredited Physical Therapy Assistant program.
• CPR certification (preferred).
• Experience in home care, Part A or B, or outpatient.
• Reliable transportation.
• A compassionate, patient-centered mindset and strong communication skills.
• Familiarity with electronic health record systems (e.g., WebPT or Hello Note).
• New Graduates are welcome to apply.

Key Responsibilities:

• Evaluate and treat patients with physical disabilities, injuries, or functional limitations.
• Develop and implement individualized physical therapy care plans.
• Provide therapeutic exercises, manual therapy, gait training, and neuromuscular re-education.
• Deliver orthopedic, post-surgical, and neurological rehabilitation services.
• Educate patients and caregivers on home exercise programs and safe mobility practices.
• Maintain documentation in electronic medical records in accordance with Medicare and CMS regulations.
• Assist with fall prevention, balance training, and pain management strategies.

While Stern at Home Therapy has become a treasure trove of white gloved care, the entire spectrum of geriatric health remains available for our patients to this very day. Although our mission is completely focused and driven by the reversal of obligation and convenience of care, our patients know we are always about helping with the bigger picture.
